About the Role
We are seeking applications from current Southwest Healthcare staff for a rewarding opportunity to join our Discharge Support & Liaison team! If you're a dedicated Registered Nurse, Occupational Therapist, Physiotherapist, Social Worker, Speech Pathologist, Podiatrist, or Dietitian passionate about supporting patients with timely and safe hospital discharge, we want to hear from you.
As a valued member of our team, you will collaborate with a multidisciplinary team to enhance patient flow, deliver optimal care, and reduce hospital readmissions. This role is your chance to play an essential part in ensuring patients receive the right care, at the right time, in the right place.
Position Overview
- Employment Type: Fixed term, Full-Time (80 hours per fortnight)
Part time applications welcomed, please specify your preferred FTE in application
- Commencing: April 2026, Ending: 20 July 2026
- Location: Warrnambool Campus
- Classification: Allied Health Professional Grade 2 or Community Health Nurse
- Salary Range: $88,464.00 - $107,390 per annum, pro rata, + superannuation
What You Will Bring
- At least 3 years of experience in your health profession
- Creativity, flexibility, and the ability to adapt to organisational changes
- Strong time management, organisation, and problem-solving skills
- The capacity to work independently while contributing to a multidisciplinary team
- Excellent interpersonal and communication skills
- Experience in subacute and community settings (desirable but not essential)
